This table outlines the end-to-end client journey at GCHC, aligned with the NSW Clinical Care Standards for Alcohol and Other Drug Treatment (2022). Each standard is represented in the participant journey with clearly defined activities, ensuring trauma-informed, culturally safe, and recovery-focused care for First Peoples. Participants can enter the programs at any stage of their individual journey, and participants are welcome to access all or part of the services offered.

Initial contact, eligibility screening, informed consent, triage, identification of urgent needs, and warm referral if unsuitable |
2. Comprehensive Assessment |
Holistic biopsychosocial and cultural assessment using validated tools; includes safety screening and formulation of initial risk and wellbeing profiles |
3. Care Planning |
Co-developed Individualised Care Plan (ICP) based on assessment outcomes, recovery goals, service needs, and cultural strengths/supports |
4. Risk Identification & Monitoring | Risk Management Plan and Safety Plan developed for at-risk clients; reviewed regularly to reflect changes in circumstances or presentation |
5. Monitoring Treatment Progress & Outcomes |
Ongoing progress tracking using ATOP, K10, WBI, care plan reviews, and client feedback; informs care plan adjustments |
6. Transfer of Care |
Coordinated discharge with consent, summary of progress and outcomes, warm referrals to ongoing care, optional 3-month follow-up |
