Confidentiality: I understand that all information shared during my participation in the program is confidential and will not be disclosed to third parties without my consent, except as required by law.
Voluntary Participation: My participation in the program is voluntary. I have the right to withdraw from the program at any time without any negative consequences. Medical Advice: I acknowledge that the program provides support and guidance but does not replace professional medical advice, diagnosis, or treatment. I will seek advice from my healthcare provider for any medical concerns.
Risks and Benefits: I understand that participation in the program may involve discussing sensitive topics and may bring up emotional or psychological issues. I am aware of the potential risks and benefits and agree to participate with this understanding. Behavioural Expectations: I agree to adhere to the programs code of conduct, which includes respectful behaviour towards staff and other participants, and compliance with all program rules and regulations.
Liability: I release the Community Healing Centre, its staff, and affiliated organisations from any liability for any harm or injury that may occur as a result of my participation, except in cases of gross negligence or intentional misconduct.
Emergency Situations: In case of an emergency, I authorise the program staff to take appropriate actions to ensure my safety and well-being, including contacting my nominated and/or emergency services if necessary.
