Thank you for taking the time to provide feedback on your experience with the Gandangara Community Healing Centre (GCHC).

Your insights help us improve our services to ensure they are culturally safe, person-centred, and responsive to the needs of our community. This form can be completed by participants, family members, carers, Elders, community members, or service providers.

 

How your feedback will be used

  • To help us learn, grow, and deliver better care and support.
  • To guide improvements in a culturally safe and respectful way.
  • Your feedback may be included (without your name) in reports to help us understand what’s working and what could be better.
  • Everything you share is kept private and secure under our privacy policies and the law.
  • If your feedback is a complaint, it will be managed under our Complaints Process. The Practice Manager will acknowledge your complaint, ensure it’s documented, and oversee a fair resolution. You may be contacted to provide more information or discuss outcomes.
